Beneficiary Agency Forms:

Agencies that provide direct services to patients/clients, provide HIV/AIDS education or prevention programs, or have innovative programs to stop new HIV infections should apply for a grant from the Foundation.  There are some changes to the way grants will be issued for the 2008/2009 cycle.  Please review the grant guidelines. 

Highest priority is given to prevention/education programs and direct client services.  Additionally, programs focusing on the current risk group (age 15-24) are also favored by the Foundation.  Site visits will not be performed for returning beneficiary agencies unless a major change has occurred or specifically requested of the Beneficiary Committee.
